Entry Deadline June 15, 2012

1) Write in your favorite choice for the categories listed, voting for at least 20 categories per ballot. Include complete name, town of establishment, person or place for which you are voting (ie: Grocery Store, Location). Votes must appear on the official ballot form as it appears in The Times Herald.

2) Please print clearly. The entire entry blank & ballot, including voter information and voter signature, must be filled out and completed by voter only. Ballots not meeting this criteria will be disqualified. All questionable entries will be verified.

3) Ballots must be completed on the Official Ballot form. No carbon or mechanical copies will be accepted. NO STAPLES! No purchase necessary. Newspapers can be examined in The Times Herald lobby and most public libraries. Ballots will be published in The Times Herald June 2nd, 3rd, 6th & 7th and on www.timesherald.com from June 7th through June 15th.

4) All ballots must be received June 15th. Ballots must be mailed or delivered individually to The Times Herald by each voter. Mail or drop ballots to: Readers Choice, The Times Herald, 410 Markley St., Norristown, PA 19401.

5) Employees of The Times Herald, Journal Register Co., Inc., their agencies and immediate families are not eligible for prizes.

6) All of the above rules will be strictly enforced. The Times Herald will be the sole and final judge of ballot eligibility and any and all questions arising from this promotion. The Times Herald reserves the right to eliminate any votes that are fraudulent, suspicious or submitted in inappropriate categories.

7) The winners of each category will be announced July 29, 2012 in The Times Herald Readers Choice special feature.
